Newton Creek Clinic has been recognized as a Tier 5 Patient-Centered Primary Care Home by the Oregon Health Authority.
The Patient-Centered Primary Care Home program is part of the Patient-Centered Primary Care Home Program and sets the standard for how primary care clinics across Oregon care for their patients.
Tier 5 is the highest level of recognition a clinic can receive.
Our recognition is valid from December 19, 2025, through December 18, 2027.
