Pathways Program

Umpqua Health is committed to improving the health of our patients and our community by providing high quality care. We want to make sure that everyone in our community has access to the care they need when they need it, and that’s why we’ve established the Pathways program. Pathways is a financial assistance program that offers discounted care to patients who cannot afford medical bills. Quit for Life helps OHP members learn to life without tobacco. Trained experts will help you put a personalized quit plan together, plus receive free quitting aids. You will have expert support and assistance whenever you need it, over the phone, from Coaches who specialize in helping people quit tobacco. TRANSPORTATION

UTrans

UTrans is a public transportation service operated by United Community Action Network that serves the greater Roseburg area, with services to Winston, Sutherlin, Myrtle Creek and Canyonville. UTrans has a bus stop right outside the Umpqua Health – Newton Creek clinic with multiple bus lines offering rides. For more information about UTrans, please visit: www.umpquatransit.com

COMPLAINTS AND GRIEVANCES

A complaint or grievance can be made electronically, in person, by phone or mail. Patients calling the clinic should ask for the Patient Navigator, send electronically to ClinicFeedback@umpquahealth.com or use this printable Patient Complaint Form for mail.  The Patient Navigator will be in touch within seven (7) days of receiving the complaint, and take action to find a resolution based on the type of issue within (30) thirty days.

PREPARING FOR YOUR APPOINTMENT

  • Please arrive 15 minutes prior to your scheduled appointment time.
  • Don’t forget to bring your insurance cards, ID and any applicable medical information. If you have any questions about preparing for your visit you can contact our scheduling team at (541) 229-7038.
  • Any policies or procedures that are critical to the patient are on this website, but patients can request a hard copy version as well.

APPOINTMENT CANCELLATIONS AND LATE ARRIVALS

  • Patients of UHNC should cancel appointments 24 hours in advance of the scheduled times when possible.
  • Patients who arrive to their scheduled appointment late and cannot be seen by the provider on the same day will be rescheduled for the next available future clinic visit.
